1. Call 911 Immediately

2. Take Photos and Video

• Document cars and accident scene.

3. Identify Any Witnesses

• Get names(s), address(es) and phone number(s).

4. Write Down Crucial Information

  • Address
  • Phone number
  • License number
  • Date of birth

• From vehicle registration:
  • License plate number
  • Year, make and model of car
  • Insurance company name

5. Injured? Go to An Emergency Room Right Away

• Or contact your doctor to make an appointment
